Incident Cause Analysis Method (ICAM) is a consistent, proven methodology that can determine contributing factors by identifying deficiencies within an organization’s systems and processes that result in a major incident.
The focus of the investigation is at an organizational systems level and includes both cultural and behavioural aspects, if applicable, where operational adherence to the application of process may, or may not, have contributed to the incident.
