OHCOW Privacy Statement

OHCOW is committed to ensure the privacy of all individuals and their personal information, regardless of how it is collected or what format it is held in. This includes any information gathered through our website.

This Privacy statement summarizes our policy and our practices as it pertains to the collection, use, transfer, storage and disposal of personal information collected through electronic means. Such information may include name, title, company name, union association, advocate name, address, e-mail, telephone and fax number. Whenever this information is sought, the intended will be clearly spelled out. If you leave this information for the purpose of contacting us or requesting information, we will keep a record of that correspondence for internal data collection purposes. You may choose not to provide the information. To do so may jeopardize our ability to service your request.

Use, Disclosure, Storage and Transfer:
OHCOW will identify the purpose and use of any personal information collected, at the time of collection.
No personal information will be disclosed to a third party without the express permission of the individual.
Storage of such collected data will only be for as long as the information is required. After the intended use has been executed, the information will be disposed of.
Information may be transferred to one of our clinics, so better to address your requests. No transfers to third parties will occur without your permission.

Access, Modifying or Removing Information:
You may at any time make a written request to find out what personal information OHCOW maintains, what the use of that information was and what the status of the stored information is.
You may also modify or remove your personal information at any time, through written request.

Data Security and Integrity:
OHCOW takes every reasonable precaution to protect information from loss, misuse and unauthorized use. All personal information will be removed and disposed of once it is no longer required for its intended use. OHCOW cannot however, guarantee that unauthorized use will never occur ( e.g. that someone will not overcome our security measures ), and makes no representations or warranties to this effect.

OHCOW reserves the right to change our Privacy policy as the need arises. In the event a change occurs, it shall be posted as a revision to the policy.
